We greatly appreciate the thoughtful letter to the editor by Shabih Manzar in response to our paper [1, 2]. Below, we provide our reply to his comments.

In regards to the first comment, it is true that fewer mechanical ventilation days in the early transpyloric feeding (TPF) group may be the causative factor associated with reduced incidence of bronchopulmonary dysplasia (BPD). However, the fact that the TPF group had significantly fewer infants that were never intubated at baseline (19% vs. 33%, see Table 1 in ref. [1]) suggests that the TPF group likely had more severe pulmonary disease after birth. Of course, we can only provide our best guess regarding causality without data from a randomized controlled trial.
