Mid-aortic syndrome in pregnancy

Hypertension is the most common and challenging medical problem during pregnancy. It complicates up to 15% of pregnancies and is a leading cause of maternal and fetal morbidity and mortality.1 Hypertension during pregnancy is classified as preeclampsia–eclampsia, gestational hypertension, chronic hypertension and superimposition of preeclampsia on chronic hypertension.2 Severe hypertension requires treatment to prevent cerebrovascular, cardiac and renal complications in the mother.3 Moreover, secondary causes should be investigated in cases resistant to drug therapy.

Mid-aortic syndrome (MAS) is a very rare cause of secondary hypertension and is characterized by narrowing of the abdominal aorta, and commonly involves the visceral and renal arteries. It is usually diagnosed in children or in young adults. Hypertension is the most common presentation of MAS. However, manifestations of congestive heart failure, oliguric renal failure, claudication and intestinal ischemia are also possible.4 Magnetic resonance or CT angiography or angiography can be used in diagnosis. Treatment options include percutenaous techniques, if feasible, and surgery.
