False positive Belin-Ambrosio enhanced ectasia screening in small white-to-white diameter

Corneal tomographic analysis is the most important diagnostic tool for screening refractive surgery candidates. The Belin-Ambrosio Enhanced Ectasia display (BAD) map was developed to differentiate early keratoconus (KC) from normal eyes [1]. The final “D” value is calculated by a regression analysis that weights various corneal parameters [1]. In the Pentacam display, each parameter (Df, Db, Dp, Dt, Da) is displayed in yellow when the value is > 1.6 SD from the mean of the normal population, and in red, when the value is >2.6 SD from the mean of the normal population [2]. In general, if the D-value is abnormal, it is indicative of an abnormal cornea. Recently, a study showed that small corneal diameters might influence the BAD display and show abnormal values even in an otherwise normal cornea [3].
