Comment on “Development and validation of a multivariable model for prediction of malignant transformation and recurrence of oral epithelial dysplasia”

We commend Mahmood et al. [1] for providing a novel insight into the progression of oral epithelial dysplasia (OED) by identifying new histological predictors not routinely considered in diagnostic practice. We take this opportunity to discuss a few aspects that have direct implications for interpreting the study’s results.

The authors state that the perimeter of the OED epithelium is associated with malignant transformation, suggesting the importance of epithelial thickness [1]. We are sceptical about the generalisability of this crucial finding. Oral submucous fibrosis, an oral potentially malignant disorder (OPMD) prevalent in Asian and Southeast Asian countries, has the highest malignant transformation rate of 4.2% [2]. This pathology is characterised by severe epithelial atrophy and the loss of rete ridges. Reduced epithelial thickness and a high malignant transformation rate in oral submucous fibrosis contradict the findings of the present study. We believe that integrating the ‘type of OPMD’ parameter will improve prognostic models in future studies.
