Thanks to advances in neonatal-perinatal care, over the past two decades more extremely-low-gestational-age (ELGA) neonates, especially those born at the fringes of viability (22–24 weeks of gestation), are surviving. Much of this success may be attributed to improvement in our understanding of pathophysiology of ailments unique to these infants, miniaturization of equipment such as vascular catheters, noninvasive means of monitoring and treatment, and enhanced attention to nutrition, infant environment, and growth. The downside of this success, however, is the emergence of morbidities among the survivors, some with effects lasting long into adulthood.
