Screen for GER and identify patients with clinically silent GER (see Table 2)
Begin 3-month empiric trial:
  - Lifestyle modifications
  - Twice daily proton pump inhibitor (PPI) 30–60 minutes before breakfast and dinner
  - Consider adding prokinetic agent
Reassess cough response in 3 months
Order esophageal manometry and pH for nonresponders on GER therapy
Examine for other causes of chronic cough in nonresponders
Consider combined esophageal pH and impedance testing in nonresponders on medical therapy
Reduce PPI to once daily after cough resolution; some patients respond to H2-blockers
