Screen for GER and identify patients with clinically silent GER (see Table 2) |
Begin 3-month empiric trial: |
- Lifestyle modifications |
- Twice daily proton pump inhibitor (PPI) 30–60 minutes before breakfast and dinner |
- Consider adding prokinetic agent |
Reassess cough response in 3 months |
Order esophageal manometry and pH for nonresponders on GER therapy |
Examine for other causes of chronic cough in nonresponders |
Consider combined esophageal pH and impedance testing in nonresponders on medical therapy |
Reduce PPI to once daily after cough resolution; some patients respond to H2-blockers |
